Every family’s journey is different, but the need for clarity and continuity is universal. Our Continuum of Care ensures that no matter where a patient begins — post‑hospital rehab, home health, palliative care, or hospice — they receive seamless, coordinated support. Below is how each service fits into the full paradigm.

Post Hospital Rehab Coordination

Safe Transitions After a Hospital Stay

A safe transition home is the first step in the Continuum of Care. After a hospital or rehab discharge, an Aurora nurse visits the patient to ensure everything is in place for a smooth recovery.

During this visit, the nurse will:

  • Assess the patient’s condition, safety, and immediate needs

  • Review medications, discharge instructions, and home setup

  • Gather all physician and PCP information

  • Collect caregiver and family contact details

  • Identify what services are required to support the patient at home

This assessment allows us to determine exactly what level of support the patient needs — whether personal care, home health, palliative care, or future hospice services.

Coordinating the Referral

Once needs are identified, Aurora helps the family request the appropriate referral from the patient’s PCP or physician. We guide the process, ensure all information is ready, and support the caregiver in making the request on behalf of the patient, keeping everything compliant and streamlined.

Where This Fits in the Continuum of Care

This is often the starting point for seniors and adults recovering from hospitalization. It stabilizes the home environment, prevents avoidable readmissions, and sets the foundation for the next level of care as needs evolve.

 

Unskilled Personal Care and Companionship Services

Non‑medical support that helps patients stay safe, comfortable, and independent at home. Through our trusted caregiver partners, families can access:

  • Bathing and grooming assistance

  • Meal preparation

  • Light housekeeping

  • Mobility and fall‑prevention support

  • Medication reminders

  • Companionship and supervision

 

Coverage Facts

Medicare and Medicaid do not cover companionship or personal care services outside of hospice. These services are classified as custodial care, which is private‑pay only.

Aurora does not provide or bill for these services — but we help families connect with reliable caregiver partners when additional support is needed.

Where This Fits in the Continuum of Care

Companionship and personal care form the first layer of support, helping patients remain safe at home while reducing caregiver strain. As needs change, families may transition into Home Health, Palliative Care, or Hospice, and we guide them through each step.

 
 

Home Health Care Services

Skilled Clinical Support at Home

For patients who need medical oversight but are not yet appropriate for palliative or hospice care, we coordinate:

  • Skilled nursing

  • Wound care

  • Medication management

  • Chronic disease monitoring

  • Physical, occupational, and speech therapy

Where it fits in the continuum: This is the clinical stabilization phase, helping patients recover, maintain function, and avoid hospitalizations.

Palliative Care

Palliative care is appropriate long before hospice. It focuses on:
