You don’t have to decide alone. Our team evaluates the patient’s condition, symptoms, goals and safety needs, then recommends the most appropriate level of care within the Continuum.

Yes. Patients often transition from home health to palliative care, or from palliative care to hospice. We coordinate every step so the transition is smooth and stressfree.

Palliative care focuses on comfort and symptom management at any stage of illness, even while receiving curative treatments. Hospice is for patients who are no longer pursuing curative treatment and want comfort-focused care.

Yes. Our PostHospital Rehab Coordination helps families manage medications, followup appointments, home safety, and care planning to prevent readmissions.

Yes — but these services are not covered by Medicare or Medicaid outside of hospice. Companionship, personal care, homemaking, and sitter services are considered custodial care, which is privatepay. Aurora does not bill for these services, but we help families connect with
trusted caregiver partners who can provide them.

Respite care provides shortterm relief for overwhelmed or exhausted caregivers. It allows family members to rest, attend appointments, or take time for themselves while their loved one continues receiving safe, compassionate support.

Yes — but ONLY under the Medicare hospice benefit. Medicare covers up to five consecutive days of inpatient respite care to give caregivers a break. Outside of hospice, respite care is not covered and is privatepay.

No. Medicare and Medicaid do not cover inhome respite care at any stage. Families may choose to hire privatepay caregivers for inhome support, and Aurora can help connect you with reliable care partners.

No. Even during hospice, companionship and sitter services are not covered by Medicare. These services remain privatepay, and Aurora can help families find trusted providers if additional support is needed.

Absolutely. We provide hospice care wherever the patient lives — home, assisted living, memory care, or longterm care facilities.

We offer emotional support, education, respite care (when covered), 24/7 nurse availability, and guidance through medical and endoflife decisions.

Yes. We communicate directly with physicians, specialists, and facilities to ensure continuity and avoid conflicting care plans.

If a patient stabilizes or improves, they may be discharged from hospice and return to palliative care or home health. The Continuum of Care allows us to adjust seamlessly.
