Our Interdisciplinary Leadership Team

Provides clinical oversight and ensures care aligns with best practices, patient goals, and the needs of families across the entire Continuum of Care.

Registered Nurses

The clinical foundation of Aurora’s care model. Our nurses manage symptoms, coordinate care, educate families, and support patients through every transition — from early symptom changes to end‑of‑life comfort.

Certified Home Health Aides

Support daily comfort and dignity through personal care, mobility assistance, and hands‑on support. Their presence helps stabilize the home environment and reduces caregiver strain.

Social Workers

Guide families through emotional, practical, and logistical challenges. They provide counseling, resources, and clarity during difficult decisions, ensuring families feel supported at every step.

Chaplains & Spiritual Counselors

Offer spiritual and emotional support for individuals of all faiths and backgrounds. They help families find peace, meaning, and comfort throughout the care journey.

Bereavement Counselors

Support families for up to 13 months after a loved one passes. They provide grief resources, guidance, and compassionate follow‑up to help families heal.

Administrative & Care Coordination Team

Ensure seamless communication, scheduling, and coordination with physicians, hospitals, rehab centers, and long‑term care facilities. Their work keeps the Continuum of Care connected and stress‑free for families.
